When are you most fertile? Ovulation and your fertile window, explained

Your fertile window is about six days: the five days before ovulation plus ovulation day itself, and the two or three days just before ovulation matter most. Having sex every one to two days through that window is what the evidence supports, with no need to save up. Dr Mayur Pai explains how to find it.

Why your fertile window matters

If you are trying to conceive, there is one thing worth understanding better than almost anything else: your fertile window. Getting the timing right is one of the simplest things you can do for yourselves each month, and it costs nothing. Yet many couples we meet have never had it explained simply. Here is how Dr Mayur Pai explains it.

What happens at ovulation?

Roughly once in each cycle, one of your ovaries releases an egg. That is ovulation. The egg is picked up by the fallopian tube, which is where it normally meets the sperm.

Two simple facts decide everything that follows. The egg lives for only about twelve to twenty-four hours after it is released. Sperm, on the other hand, can survive inside the woman's body for up to about five days.

How long is the fertile window?

Put those two facts together and something important falls out: your fertile window is not a single day. It is roughly the five days before ovulation, plus the day of ovulation itself — about six days in total.

Within that window, the two or three days just before you ovulate are the golden ones. Sperm that arrive a little ahead of the egg are already waiting when it is released. That is why the days leading up to ovulation matter more than the days after it.

How can you tell when you are ovulating?

There are a few honest, practical ways to find your window. None needs to be perfect; used together, they give you a good picture.

Tracking your cycle. If your periods are fairly regular, ovulation usually happens around the middle of the cycle. But the idea that it always happens on day fourteen is a myth, because cycles genuinely vary from woman to woman. In many women, ovulation comes roughly two weeks before the next period is due, so in a longer cycle it tends to come later, and in a shorter cycle earlier. Noting the first day of each period for a few months shows you your own pattern.

Ovulation predictor kits. These kits, available at any pharmacy, detect the hormone surge that comes a day or so before you ovulate. They are genuinely useful, especially if your cycle length varies a little. A positive result tells you ovulation is likely to follow soon, which makes that day and the next a good time to try.

Your body's own signs. Around ovulation, the cervical mucus changes. It becomes clear, slippery and stretchy, a little like raw egg white. That is your body quietly telling you the window is open.

How often should you have sex when trying to conceive?

This is the part couples most often get wrong. You do not need to save up, and you do not need to wait for one perfect day. The evidence here is clear and, honestly, reassuring: having sex every one to two days across the fertile window gives the egg and sperm the most opportunities to meet.

Saving up for days does not help. Regular sex keeps healthy sperm in place, so that whenever the egg is released within the window, sperm are already there to meet it.

Keep it regular and relaxed, and do not turn it into a stressful project on a spreadsheet. Pressure and anxiety help no one, least of all your chances.

What if your periods are irregular?

If your periods are very irregular, ovulation may be genuinely unpredictable, or may not be happening every month. That is not something to simply live with; it is worth getting checked. Common conditions such as PCOS or a thyroid imbalance are often behind it, and they are very treatable once they are found. A scan and a few blood tests usually show what is going on. Read more about PCOS and what the ultrasound shows.

What can good timing not fix?

Perfect timing improves your odds, but it cannot overcome a blocked tube or a significant sperm problem. If the tubes are blocked, the egg and sperm cannot meet, however well the timing is planned. If the sperm count is very low, far fewer sperm reach the egg in the first place. Neither problem shows up on a calendar.

That is why, if several months pass despite good timing, a proper evaluation is the kind and sensible next step, rather than simply trying harder on your own. As a general guide, see a doctor after about a year of trying, or after six months if the woman is over thirty-five, and sooner if periods are very irregular. Both partners should be checked together: read about the common causes of infertility and about male infertility.

Timing, without turning intimacy into a chore

Understanding your window is not about turning intimacy into a chore. It is simply knowing when your body is most ready, and then letting nature have its fullest chance.

Where treatment happens

This guide is by Dr Mayur Pai, MBBS, MS (Obstetrics & Gynaecology), a fertility and laparoscopy specialist. Pai Hospital in Goa has its own operating theatre and its own IVF laboratory, so if tests or treatment are needed, the scans, the laboratory work and any procedures happen in one place, under one team. Your questions

How many days is the fertile window?

About six days in each cycle: the five days before ovulation and the day of ovulation itself. The two or three days just before ovulation are the most fertile, because sperm can survive for up to about five days while the egg lives for only about twelve to twenty-four hours.

Is ovulation always on day fourteen?

No, that is a common myth. In fairly regular cycles ovulation usually happens around the middle of the cycle, but cycles vary from woman to woman, so your fertile days may come earlier or later. Ovulation kits and the change in cervical mucus help you find your own window.

How often should we have sex when trying to conceive?

Every one to two days across the fertile window. There is no need to save up or to aim for one perfect day. Keep it relaxed, because pressure and anxiety help no one.

When should we see a doctor if our timing is right?

If several months pass despite good timing, get checked together as a couple. As a general guide, see a doctor after about a year of trying, or after six months if the woman is over thirty-five, and sooner if periods are very irregular. Good timing cannot overcome a blocked tube or a significant sperm problem.

Where can we get checked in Goa, and is travel help available?

Consultations are available across Goa, and tests and treatment take place at Pai Hospital in Goa, which has its own operating theatre and its own IVF laboratory. Free patient transport is available from across Goa and nearby areas.
