How many IUI attempts should you try before moving to IVF?
For many couples, IUI is worth a limited number of well-timed attempts, usually three or four, before IVF is discussed. If IUI is going to work, it usually does so within those first few cycles. Your number may be smaller, depending on the wife's age, the egg reserve, the sperm report, the cause and how long you have been trying.

In this video, Dr Mayur Pai discusses the usual upper limit for IUI and the factors that decide when a couple should consider a different treatment. It is one of the questions couples ask most, because nobody wants to give up too early, and nobody wants to lose precious months either.
Why is there a limit at all?
IUI helps in specific ways. It places prepared sperm high in the womb at the right time, often in a cycle where ovulation is supported with medicines. But it cannot open a blocked tube, it cannot show whether the sperm and the egg are able to fertilise, and it cannot make up for a very low egg reserve. If several well-timed attempts, with good follicles and a well-prepared sample, have not worked, the chance that the next identical attempt will be different becomes smaller.
Each attempt also costs time, money and emotional energy, and for older women, time is the most precious of the three. A limit is not about giving up. It is about moving, while time is still on your side, to a treatment that can do more.
What decides your number?
- The wife's age. A younger woman can usually complete the full course. From the mid-thirties, fewer attempts are usually advised before moving on, and around forty or later, IVF is often discussed early; see fertility treatment after 40.
- The egg reserve. With a low AMH, spending many months on IUI is rarely wise; see low AMH explained.
- The cause. IUI suits ovulation problems such as PCOS, a mild male factor and unexplained infertility. It is not suitable when both tubes are blocked, and it is usually not the right tool for a severe male factor, where IVF with ICSI is preferred.
- The sperm after preparation. If the number of active sperm after washing is repeatedly low, the doctor may advise moving on sooner.
- How long you have been trying. A couple who has already tried for many years may be advised fewer IUI attempts.
- How the cycles went. An attempt only really counts if it was a good one: follicles monitored by scan, the trigger timed, and a properly prepared sample.
When is it time to move on sooner?
The doctor may suggest discussing IVF before you reach the usual limit if:
- the wife is older, or the egg reserve is low
- cycles keep being cancelled because too many or too few follicles grow
- the prepared sperm sample is repeatedly weak
- a new problem is found, such as endometriosis or a problem with the tubes
- you both feel worn out by repeating the same thing
The last point matters. How you feel is a valid reason to change course.
What should be reviewed after each attempt?
Each cycle that does not work should be looked at, not just repeated. Did the follicle grow well and release its egg at the expected time? Was the lining good? How was the prepared sample? Were the medicines right for your ovaries, or should they be changed? Were all the basic tests done before starting, including a check that the tubes are open? Sometimes a small change in the next cycle makes sense. Sometimes the review itself shows that IUI is not the right tool.
Does a failed IUI mean IVF will not work?
No. IVF works differently. The eggs are collected directly from the ovaries, and egg and sperm are brought together in the laboratory, so the tubes are no longer needed and fertilisation can be helped with ICSI if necessary. Many couples worry that moving to IVF means their IUI attempts were wasted. They were not. They answered a question, and that answer points to the next step.
Can we take a break between attempts?
Yes. IUI cycles can often be done in consecutive months, once a baseline scan confirms that the ovaries have settled. But there is no rule that you must. If you need a month or two to rest, that is fine, as long as the break does not stretch too long when age or a low reserve means time matters. Talk it over with the doctor.

Can IUI be done in back-to-back months?
Usually yes, if a baseline scan at the start of the next period shows that the ovaries have settled. Taking a month off is also fine, as long as time is not a concern for you.
Does a failed IUI mean something is wrong with us?
Not necessarily. Even in a well-timed cycle, pregnancy does not happen every month, so more than one attempt is usually needed. If several attempts do not work, it is a signal to review the plan, not a verdict.
Should we go straight to IVF without trying IUI?
Sometimes, yes. When both tubes are blocked, the sperm count is very low, or the wife's age and egg reserve mean time is short, IVF may be the more sensible first step.
Is IVF much harder than IUI?
It is more involved: daily injections, egg collection under light anaesthesia and an embryo transfer. But each step is explained, most women manage it while carrying on with daily life, and the team is with you throughout.
