Can IUI help when sperm motility or morphology is poor?
When sperm swim poorly or many are abnormally shaped, IUI can help in milder cases. The sample is washed so that a concentrated group of the most active sperm is placed high in the womb at ovulation. If very few good swimmers remain after washing, or well-timed IUIs have not worked, IVF with ICSI is usually the better route.

What do poor motility and poor morphology mean?
Motility is how well sperm swim. When motility is poor, called asthenozoospermia, fewer sperm swim forward with purpose. More of them twitch on the spot, spin in tight circles or do not move at all. Forward movement is what counts, because sperm have to travel through the cervix and the womb to meet the egg in the tube.
Morphology is the shape of the sperm. When morphology is poor, called teratozoospermia, fewer sperm have the normal oval head, neat midpiece and single straight tail. The standard for "normal" is strict, so this figure looks low on most reports; it matters most when it comes together with other problems. How does IUI help sperm that swim poorly?
In natural conception, sperm must cross the acidic vagina, get through the mucus of the cervix and swim the full length of the womb. Weak swimmers fall behind at every stage, and only a small number arrive near the egg.
IUI shortens that journey. On the day, the sample is washed in the laboratory: the seminal fluid, dead and sluggish sperm, other cells and debris are removed, and the active sperm are gathered into a small drop of clean fluid. That drop is placed high in the womb through a thin, soft tube, timed with scans and a trigger injection to the moment the egg is released. Does IUI help abnormally shaped sperm?
Here honesty matters. Washing tends to pick out the more active sperm, and active sperm are more often well formed, so the prepared sample usually looks better than the raw one. But IUI does not change the shape of any sperm. It only changes which sperm get the head start.
When poor shape is the only finding and the count and movement are reasonable, IUI can be a sensible thing to try. When poor shape comes together with poor movement and a low count, IUI becomes less likely to work, and IVF with ICSI is discussed earlier. What is checked before choosing IUI?
- A repeat semen analysis. Motility dips after a fever, after a long gap without ejaculation, or when a sample cools before it reaches the laboratory, so one poor result is confirmed first.
- A trial preparation. Sometimes the sample is washed in advance to see how many good swimmers can be recovered. That tells us more about IUI than the raw report does.
- Treatable causes. Pus cells in the semen may point to an infection, which is treated first. A varicocele, hormone problems, smoking, heat and heavy alcohol are also looked for.
- The wife's side. IUI needs at least one open tube and an egg released on time, so her tests are done alongside his.
When is IVF with ICSI the better route?
IVF with ICSI is usually advised when:
- only a few moving sperm are recovered after washing
- poor movement, poor shape and a low count all come together
- a few well-timed IUI cycles have not led to a pregnancy
- the wife's tubes are blocked, or her age or egg reserve means time is short
In ICSI, the embryologist chooses a single sperm that is moving and looks healthy, and places it directly inside each egg. The sperm no longer has to swim to the egg or push its way in, which is why ICSI suits exactly the problems that make IUI struggle. Can motility and morphology improve?
Sometimes. Sperm take about two to three months to form, so changes made today show on a test a few months later. Stopping smoking, cutting down alcohol, keeping the testes cool (no laptop on the lap, no very hot baths), reaching a healthy weight, sleeping well and treating any infection can all help. These steps are worth taking, but they should run alongside treatment rather than delay it, especially when the wife's age is a concern.

Can IUI work if my sperm motility is low?
It can, when motility is mildly or moderately reduced and enough active sperm are recovered after washing.
