Which early pregnancy signs are normal, and which are warnings?

Tiredness, nausea, tender breasts, needing to pass urine more often and mild cramps are common and normal in early pregnancy. Severe lower tummy pain, especially on one side, heavy bleeding, fainting, fever, or vomiting that stops you keeping fluids down are warnings that need a doctor the same day. An early scan confirms where the pregnancy is.

Why do early pregnancy signs feel so confusing?

When you have just found out that you are pregnant, or you suspect you might be, your body can suddenly feel unfamiliar. Every twinge and every change can make you wonder whether something is wrong. Early pregnancy brings many new sensations, and most of them are simply signs of your body adjusting to the pregnancy hormones. A few, however, are warnings that need prompt medical care. Knowing the difference helps you stay calm when things are normal, and act quickly when they are not. If you are feeling anxious, that is understandable; many women feel the same in these first days, and asking questions is always the right thing to do. What are the normal early signs of pregnancy?

A missed period is often the first sign, although women with irregular cycles, such as those with polycystic ovary syndrome, may not notice it straight away. Tiredness is very common and can feel overwhelming. Nausea, sometimes with vomiting, is also common, and despite the name, morning sickness can happen at any time of day. Your breasts may feel tender, heavy or fuller, and the skin around the nipples may darken. You may need to pass urine more often, feel bloated, notice mild cramps rather like a period coming, find certain smells or foods suddenly unpleasant, or feel more emotional than usual.

These symptoms vary widely. Some women have many of them and some have hardly any, and both can be completely normal. Having few symptoms does not mean something is wrong, and having strong symptoms does not mean something is wrong either. What matters is noticing the few signs that are different. Resting when you can, eating small and frequent meals, and drinking enough water often help with tiredness and nausea.

Is spotting in early pregnancy normal?

A little light spotting can happen in early pregnancy and is often harmless. However, bleeding is also one of the signs that needs checking, because it can occasionally be linked to a miscarriage or to a pregnancy outside the womb. So if you notice any bleeding, it is always worth telling your doctor. Spotting after sex or after an internal examination can also happen, because the neck of the womb has a richer blood supply in pregnancy, but it is still worth mentioning. If the bleeding is heavy, you are passing clots, or it comes with pain, please seek care the same day rather than waiting to see if it settles.

Which signs are warnings that need a doctor today?

Please seek medical care the same day, without waiting for a routine appointment, if you have any of the following: severe pain in the lower tummy, especially if it is on one side; heavy bleeding or passing clots; fainting, feeling dizzy or light-headed, or pain at the tip of your shoulder; a fever or a bad-smelling discharge; or vomiting so severe that you cannot keep even water down, or you are passing very little urine. These signs do not always mean something serious, but they always deserve a prompt examination. Pain or burning when passing urine also needs a prompt check, because infections should be treated early in pregnancy.

Why is one-sided pain so important?

Sometimes a pregnancy settles outside the womb, most often in one of the fallopian tubes. This is called an ectopic pregnancy. It cannot continue normally, and as it grows it can cause the tube to burst, which can bring on serious internal bleeding. An ectopic pregnancy often gives very few warnings in the early days, and the pain may be mild at first. Severe or one-sided pain, pain at the tip of the shoulder, or fainting can be signs of internal bleeding and need emergency care. This is also why an early scan matters so much, even when you feel well. If you have had an ectopic pregnancy before, or surgery on your tubes, mention it to your doctor straight away, as an early scan is even more important for you.

Why does an early scan matter?

An early ultrasound scan gives you and your doctor facts rather than guesses. It confirms that you are pregnant and that the pregnancy is inside the womb, where it should be. It shows how the pregnancy is developing and helps date it accurately, which matters for every decision that follows. If your periods are irregular, or you are unsure when your last period was, the scan is especially helpful in working out how far along you are. It can also show a pregnancy that has stopped developing, sometimes before there are any symptoms. Whatever you are thinking about, whether you hope to continue this pregnancy or you are unsure, the scan is the safe first step.

What if you are not sure about continuing the pregnancy?

If you are unsure whether to continue, you are not alone, and you do not have to decide on your own. In India, abortion is lawful care under the Medical Termination of Pregnancy Act, provided at a government-licensed hospital by a qualified doctor. Unmarried women are just as welcome, and every woman is treated the same, with privacy and respect.
