Why does a positive test feel so frightening?

You have had an abortion and felt that it was behind you, and then a home test still shows positive. It is natural for your heart to sink, and to wonder whether the abortion has worked, or whether you are still pregnant. Many women go through this moment quietly and alone. We want to reassure you: a positive test after an abortion is often expected, and on its own it does not mean that anything has gone wrong. Below, we explain why, and when you should contact us. What does a home pregnancy test actually measure?

A home pregnancy test does not see the pregnancy itself. It detects a hormone that the pregnancy produces, which passes into the blood and then into the urine. Modern home tests are very sensitive and can pick up even small amounts of this hormone. That sensitivity is helpful when you are trying to find out whether you are pregnant, but it also means that a test can keep reacting to the hormone that remains after a pregnancy has ended. A faint line and a dark line both mean only that some hormone is present. Neither can show what is happening inside the womb.

Why does the hormone take time to clear?

After an abortion, the pregnancy hormone does not disappear overnight. The level falls gradually, over some time, and the pace differs from woman to woman, depending partly on how much hormone was present to begin with. During this time, a home test may stay positive, or show a line that becomes fainter as the days pass. This is part of the body settling, just as bleeding and cramps settle. Many women also notice that pregnancy symptoms, such as nausea or tender breasts, fade gradually over the same period, which is a reassuring sign. Testing again and again at home usually adds worry without adding answers, because a faint line cannot tell you whether everything is complete. How long can a test stay positive?

There is no single timeline that fits every woman, which is why we do not give a fixed number. How long a test stays positive depends on how much pregnancy hormone was present before the abortion, and on how quickly your own body clears it. For some women the test turns negative fairly soon; for others it takes longer, and both can be completely normal. What matters is not the exact day, but whether the level is steadily falling and whether the womb is empty. At your follow-up visit, the doctor can tell you what to expect in your own situation, and whether, and when, it would be useful to test again at home.

What shows the real picture?

Your follow-up visit does. At that visit, the doctor asks how you are feeling, how your bleeding has been, and whether any pregnancy symptoms have continued. An ultrasound scan then shows whether the womb is empty, which is the question that really matters. Sometimes a blood test for the pregnancy hormone is done and repeated a few days later, to confirm that the level is falling as it should. Together, these give a clear answer that no home test can. How does the way the abortion is done help?

When a surgical abortion is done by suction or a D and C, it is carried out under real-time ultrasound guidance: the doctor watches the womb on a screen throughout. This lowers the risk of complications such as perforation, a small tear in the wall of the womb, and it shows the exact moment the womb is empty. The procedure stops right there, with no unnecessary scraping (over-curettage), which helps protect the lining of the womb for future pregnancies. Every instrument is single-use and disposable, opened fresh from its sealed pack for you and never used on anyone else, so there is no cross-contamination between patients.

With a medical abortion, there is no procedure, and the follow-up visit and scan are how we confirm that the medicines have done their work.

When should you call us?

Please contact us without waiting for your follow-up if you still feel pregnant, with continuing nausea or breast tenderness; if you had very little bleeding, or none at all, after a medical abortion; if you have heavy bleeding, severe or one-sided pain, a fever, a bad-smelling discharge, or you feel faint; or if your test is still positive after the time your doctor mentioned. These signs can mean that some pregnancy tissue has remained, that there is an infection, or that something else needs attention. Each of these can be checked simply, and if a small procedure is ever needed to complete things, it is done with the same care described above. If you are ever unsure whether something is normal, it is always fine to call and ask; we would much rather hear from you early.

Could it be a new pregnancy?

It is possible. Fertility can return quickly after an abortion, often before your next period arrives. If a test became negative and later turns positive again, or you had sex without contraception after your abortion, a new pregnancy is one explanation that your doctor will consider.
