Low AMH and a low sperm count together: what can be done?
When the woman has a low AMH and the man has a low sperm count, the plan has to consider both at once. Low AMH means time matters, and the sperm result decides which treatment is suitable. Why this combination needs a joint plan
Many couples hear about one problem at a time: the wife's AMH on one visit, the husband's semen report on another. When both results are low, looking at them separately can waste precious months. In the video, Dr Mayur Pai talks through this situation, which he sees often, and explains why the two results have to be read together.
A low AMH usually means the egg reserve is smaller than expected for the woman's age, so there is less time to try treatments one after another. A low sperm count affects which treatment has a fair chance of working. Together, they point towards a plan that does not delay.
What a low AMH means
AMH is a blood test that gives a rough idea of how many eggs are left in the ovaries. A low result means a smaller reserve, not that pregnancy is impossible, and it says nothing about egg quality. Age remains the most important factor for egg quality. Read more about IUI when AMH is low.
What a low sperm count means
A semen analysis looks at the number of sperm, how well they move and their shape. A single low result should be repeated, because counts vary from one test to the next, and something as simple as a recent fever can lower them for a while. Common causes include swollen veins around the testis (varicocele), hormone problems, infections, smoking, heavy alcohol, heat exposure and extra weight; sometimes no cause is found. The tests that shape the plan
- For the woman: AMH, an ultrasound count of the small follicles in the ovaries, a check that the tubes are open, and basic hormone and thyroid tests.
- For the man: a repeat semen analysis and, where needed, hormone tests and an examination to look for a treatable cause.
Treatment options
Improving the sperm where possible. Stopping smoking, cutting down alcohol, avoiding heat, reaching a healthy weight and treating any cause that is found can all help the sperm. Sperm take a few months to develop, so the effect is not immediate. That is why this runs alongside the woman's treatment, not instead of it, when her reserve is low.
IUI with prepared sperm. In milder cases, if the woman's tubes are open and enough good sperm remain after the sample is washed and prepared in the laboratory, IUI may be tried for a short while. How sperm is prepared for IUI.
IVF with ICSI. When the sperm count is very low, or when time is short because of a low AMH, IVF with ICSI is often the more direct route. In ICSI a single sperm is injected into each egg in the laboratory, so fertilisation does not depend on the number of sperm.
